Each curriculum committee chair will submit a report to Academic Affairs prior to each AACC meeting on diskette for distribution to council members. A link to the template is provided below. In the body of the report list the proposal number, then under item list the prefix, course number and title or major/program, the nature of the request i.e., such as name change, curriculum change etc., list what the recommendation is for the proposal, and finally if there are any comments, please add those in the space provided. (Below is the template in Microsoft Word format for curriculum committee chairs use when preparing their report for each AACC meeting. Curriculum Committee chairs may either submit their report to Academic Affairs via email two days prior to each AACC meeting for distribution to council members or bring the diskette along with hard copies for council members to the AACC meeting. Reports should be submitted to Vickie Garlitz in Academic Affairs at vgarlitz@ucok.edu. Please add your committee number and the date where provided on the report form. EXAMPLE:
